Information

Scheduling and Office Hours

  • Office hours are as follows:

    • Monday, Tuesday and Thursday: 8:00-5:00 p.m.

    • Wednesday and Friday: 8:00-2:00 p.m.

  • Walk-in appointments are not available.

  • Because demand for new patient appointments is so high, and because wait times for new patient appointments with psychiatrists (especially those participating on insurance plans) have risen, we have instituted a $50 fee to hold each new patient appointment. The fee is refundable and will be applied toward future office charges once the patient arrives for his/her first appointment.

  • The fee is refundable and will be applied to future office charges once the patient arrives for their first appointment. The fee is non-refundable to patients who cancel or no-show for their first appointment.

  • Please arrive on time for your appointments. All patients expecting to be late for their appointment should notify the front desk.  Patients arriving 10 minutes past their scheduled appointment time may be asked to reschedule at the discretion of the treating provider.

  • At least 24 hours’ notice of cancelation of an appointment is expected. Notification within less than 24 hours may result in a fee. This fee is not reimbursable by insurance.

Medication Refills

  • Our policy is generally to issue enough refills at the time of a patient’s appointment to last until the next appointment.  There are exceptions.

  • A patient requesting replacement prescriptions, new electronic prescriptions because of a change in pharmacy, or new prescriptions because of an appointment cancellation or reschedule will be charged a fee for the processing of the refill.

  • Refill requests can be made in the following ways: by calling the office directly, by having your pharmacy contact us, or by messaging Dr. St John through our patient portal.  Please allow at least 24-48 hours for the refill to be processed.  Patients insisting on same-day turnaround may be charged a fee for the processing of the refill.

Preparing For Your First Appointment

  • Please download and complete forms according to your appointment reason (see below) and arrive 10 minutes prior to your appointment time with the forms completed.

  • Please plan to arrive at least 30 minutes prior to your appointment time if you plan to complete forms in the office.

  • Please bring the following items with you to your first appointment:

    • Insurance card (if applicable) and driver’s license or picture ID

    • The insured person's date of birth and social security number

    • All medications you are currently taking

    • Any medical records you have in your possession

    • A support person if desired (e.g. spouse, significant other, parent, legal guardian, friend, etc.)

    • A form of payment

    • The address and phone number of your local and mail order pharmacies

  • If a referral is required by your managed care plan, it must be received in our office 24 hours in advance of your appointment time. If we have not received the referral by that time, your appointment will be rescheduled or you may elect to pay the full self-pay fee for your visit and not have charges billed to your insurance carrier.

  • It is our policy not to complete any disability applications or paperwork at the time of an initial consultation. A separate appointment must be scheduled for a later date to discuss any issues related to a disability claim.

To Access Your Medical Record

To download portions of your medical record and/or to send secure messages to Dr. St John regarding non-urgent clinical questions or medication refill requests, go to patientonlineportal.com

First-time users will need to complete registration as directed.

If you are not able to log in, data such as your email address or SSN may be missing in our electronic medical record. Please contact the office by phone to inform us of the problem, or ask us to when you are in the office for your next appointment to input the missing information into our EHR.

To Schedule Appointments Online

Please note: This applies to active patients only who have already had at least 1 appointment with one of our providers!

Our current patients are able to schedule appointments online by accessing our providers’ profile on Kareo.com:

Martha St John, MD:  http://provider.kareo.com/martha-st-john

Liz Grizzaffi, PA-C:  http://provider.kareo.com/elizabeth-grizzaffi

When an appointment is requested online, our office staff will review the appointment request and either confirm it or respond with an alternative time. Beware that blocks we have placed for provider vacation or time off may not show up on the schedule visible by patients.

Please also note that appointments can only be canceled by a direct phone call to the office.
